Medication Administered At School

When it is necessary for medication/medical intervention to be administered while a student is at school you must complete - Administration of Medications Authorization Form and Administration of Medication Record (attached) before dropping off medication. Please do not send any medications to school with your child. Parents/guardians are requested to drop off or pick up medications at the main office.
